Please ensure that you have a general idea of: - The current health status of the individual requiring care - The current cost of care, if any, of the individual - The current monthly income from all sources for the individual requiring care - The current assets and their approximate value of the individual requiring care - Whether the individual requiring care has made any transfers of their assets in the last sixty months - Whether the individual requiring care is a Veteran, or the surviving spouse of a Veteran, and, if applicable, their dates of service
